An exciting opportunity has arisen to be involved in the delivery of Type 2 Diabetes care in NHSGGC for a Practice Nurse with a proven interest in Diabetes or a Diabetes Specialist Nurse. The successful post holder will deliver the Type 2 Diabetes education programme Control it Plus for newly diagnosed patients and will provide wider support to promote self-management and Diabetes care working closely with Diabetes Specialist Nurses and Primary Care support teams. The post will work in partnership with other Health Care professionals across the primary / secondary care interface to develop and implement programmes of specialist diabetes education for health care professionals including District Nurses, Practice Nurses and Care Home Staff. Hosted within the South Glasgow DSN team the post holder will provide diabetes management, treatment and education service to enable people with diabetes and their carers’ to effectively self-manage their condition. Working autonomously in nurse-led clinics, patient homes and community settings, assessing, planning and implementing individualised patient care plans using a person centred approach.

The Post Holder Will Be
